- Cumulative 3.0 GPA
- Prerequisite 3.0 GPA
- Minimum 'C' in prerequisite courses*
- B or better in Human Anatomy
- B or better in Human Physiology

- Statement of Interest
- 50 Observation hours
- One (1) letter of endorsement from a partner university's athletic trainer or faculty representative
